I've updated the Steps to Reproduce: it can reliably be reproduced (by me) by rolling over adjacent menu items on the WP Admin bar.
I'll work on a screencast.

@jeffpaul I think you reproduced it at least once in screencast. Rolling over the Distributor menu item may trigger it more consistently - I noticed you went around it a couple times.
